COURSE COSTS
You understand that $99.50 is non-refundable and if you cancel this registration that portion will not be returned.
REQUIRED SUPPLIES
*You will need a laptop for classroom study and access to our files; a Windows operating system is required.
*Please have the following software installed on your laptop before arriving. (They are all free, and preinstalled on many computers). If you have trouble installing any of these, don’t worry, we will help you when you arrive. We just ask to be as prepared as possible as it saves time so you can get right to learning: Microsoft Internet
*In addition, the book Troubleshooting and Repairing Major Appliances, 3rd Ed, by Eric Kleinert is required and can be purchased from Amazon.
EPA CERTIFICATION
Please obtain your EPA certification before coming to class. You are required to show proof of your certification before you can work on refrigeration products. The test can be taken at www.epatest.com/608. You will need a 608 Type I certification. Don’t worry, the test is open book and doesn’t require hands-on experience to pass.

Please bring a valid credit card for security purposes and a photo ID so to verify your identity.
EMAIL ADDRESS
Please come prepared with an email address that you can access from anywhere. This is required to register for our cloud document service which hosts thousands of service manuals and various other job aides.
FLATS ACCESSIBILITY
Once you register, please watch for another email that will supply you the code to access your living space at our flats. You will receive the code by midweek, the week before class starts. You are allowed to check in on the Sunday before class starts and check out on Saturday after class ends
FLYING TO OUR LOCATION
Primary Choice: Cleveland Hopkins Airport, Code: CLE (55 Miles from CLE to Flats)
Secondary Choice: Akron/Canton Airport, Code: CAK (84 Miles from CAK to Flats)
TRANSPORTATION
If transportation is needed from the airport to the flats (or from the flats to the airport), it is the responsibility of the student. We recommend using Shima Limo, please contact them at 888-546-6288, www.shimalimo.com prior to your arrival to schedule a pick-up. Tell them we sent you. The flats are within walking distance of the classroom and lab; is about a 3-minute walk. It is not a necessity to have a vehicle while attending our Academy, but one will make traveling to the grocery store and running errands easier.
AGE
You must be 18 years of age or older no later than the first day of class.

The classroom and lab are within walking distance of the flats. It is about a 3-minute walk. Please report to the following location each day (please plan to arrive a few minutes early so that we can start promptly at 8:00 AM):

MEALS
Fred’s Appliance Academy provides lunch Monday through Friday. Breakfast and dinner are the responsibility of the student. There are a variety of places to eat dinner in the area, as well as grocery stores if you prefer to prepare your own meals.
CLASS ATTIRE
Business casual/jeans; you may want to bring a long sleeve shirt to protect your arms. No shorts. No open shoes.
INJURY, DAMAGE RELEASE AND HOLD HARMLESS FORM
Fred's Appliance Academy, LLC maintains a training center in Ohio for the training of personnel in certain service procedures on major appliances, which include, but are not limited to, automatic washers, gas & electric dryers, gas & electric ranges, dishwashers, compactors, hot water dispensers, and refrigerators. The undersigned, for his/her own benefit, desires to participate in such training and realizes that such training is subject to inherent risk. The undersigned understands that the training received at the Fred's Appliance Academy Training Center is informational only and not accredited by any type of association or accrediting agency.
The undersigned hereby releases and agrees to defend, protect, indemnify, and hold Fred's Appliance Academy, LLC, their parents, subsidiaries, affiliates, agents, employees, officers, directors, representatives, shareholders, successors and assigns (collectively referred to as “Indemnified Parties”) harmless from and against all liability, demands, fines, claims, costs, expenses (including attorney's fees), causes of action, and suits (“Claims”) including but not limited to Claims in connection with bodily injury, death, mental anguish, and property damages, arising in favor of any person, corporation or other entity in any way related to or arising out of the training activities at the Fred's Appliance Academy Training Center. This indemnity shall apply notwithstanding the fact that any claim arises in whole or in part from the concurrent or contributory negligence of the indemnified parties. In the event that a Claim is brought hereunder, requiring defense by the below-named individual, the counsel chosen shall be approved by the Indemnified Party/Parties.
In no event shall Fred's Appliance Academy, LLC be liable for any lost or prospective profits or any other special, punitive, exemplary, consequential, incidental or indirect losses or damages (in tort, contract or otherwise) under or in respect to this agreement or for any failure of performance related hereto howsoever caused.
In addition, Fred's Appliance Academy, LLC makes no warranties or representations of any kind, oral or written, express or implied, including fitness for a particular purpose, regarding the training provided at the Fred's Appliance Academy Training Center.
PROTECTIVE EYEWEAR IS MANDATORY WHEN WORKING IN THE LAB AT ALL TIMES.
It is the student's own responsibility to ensure that they are wearing protective eyewear.
PREREQUISITES AND ELIGIBILITY
Registration is open to any adult over the age of 18; students are required to be able to lift fifty (50) pounds. Students must have Type 1 608 EPA license Students must have a laptop with a Windows operating system and Internet Explorer. Students must have the textbook Troubleshooting and
GRADING SYSTEM
Students are graded on a variety of subjects and quizzes throughout the course. This course requires an average score of 75% to pass. The following letter grade system is used: A 100 – 94, B 93 – 88, C 87 – 81, D 80 – 75, F 75 - 0
LEAVE/ABSENCE/TARDINESS/MAKEUP WORK
Because of the nature of the course, it is not possible to take a leave of absence; the course must be completed
in one 3-week setting. Students missing more than 1.5 days will not be issued a certificate from the Academy. Tardiness will not be tolerated; class will not be held up or time spent reviewing what has been missed. Make-up work will be issued at the instructor’s discretion.
CONDUCT POLICY
Fred’s Appliance Academy shall not be held liable for any injury(s) to individuals while on Fred’s Appliance Academy, Fred’s Appliance, or Gene, Sr properties or any loss(es) and/or damage(s) to an individual’s property that may occur while on Fred’s Appliance Academy, Fred’s Appliance, or Gene, Sr properties. All students are responsible for following safety codes and procedures related to the course. Students are expected to respect and adhere to school policies and course regulations. All students enrolled in any program at Fred’s Appliance Academy are expected to display good conduct and self-discipline.
Suspension or Termination and Probationary Period Policy
Students may be suspended or terminated from the Academy for reasons including unsatisfactory work, attendance or disciplinary issues. If during the first week the instructor recognizes that the student has not completed coursework, as outlined in the Satisfactory Academic Progress section, the instructor may send the student home at his or her own expense (or his or her employer’s expense). In order to be allowed re-entry into the program, students will need to reregister and seek permission of instructor. No refunds will be given for students who have been suspended or terminated.
COMPLAINT OR GRIEVANCE PROCEDURE
All student complaints should be first directed to the school personnel involved. If no resolution is forthcoming, a written complaint shall be submitted to the director of the school. Whether or not the problem or complaint has been resolved to his/her satisfaction by the school, the student may direct any problem or complaint to the Executive Director, State Board of Career Colleges and Schools, 30 East Broad Street, Suite 2481, Columbus, Ohio, 43215, Phone 614-466-2752; toll-free 877-275-4219
SATISFACTORY ACADEMIC PROGRESS (SAP)
The Fred’s Appliance Academy Basic Appliance Repair Program is 120 clock hours. Satisfactory progress is evaluated every week throughout the program.
The student is required to make quantitative progress toward program completion. To be making satisfactory academic progress, a student must attend at least 90% of the scheduled class hours on a cumulative basis during each evaluation period.
The student’s hands-on participation in the classroom and in the lab, along with completion of required reading will be used to determine the student’s qualitative progress. The minimum required is 75% at the conclusion of each evaluation period.
Incomplete grades are not given.
Students who withdraw from the program will not receive a certificate of completion. Students who withdraw from the program will not be eligible to complete the PSA and NASTEC certification tests.
ACADEMIC PROBATION
If a student fails to meet the cumulative 90% attendance or the 75% evaluation average for any evaluation period, or both, he or she will be placed on probation for the next evaluation period. Failure to achieve 90% attendance or the 75% evaluation, or both, at the end of the probation period will result in the administrative withdrawal of the student.
Students (and their employer, if applicable) will be notified in writing when they are placed on academic probation and the steps necessary to be removed from academic probation status. Students will also receive attendance or academic counseling from the School Director, as appropriate, when they are placed on academic probation.
Fred’s Appliance Academy will notify a student in person, in writing, if he or she is being administratively withdrawn for unsatisfactory academic progress. If applicable, the student’s employer will be notified by phone or email, and by certified letter.
APPEAL
The student may submit a written appeal of his or her dismissal within 2 calendar days of their receipt of the dismissal notice. The appeal must be accompanied by documentation of the mitigating circumstances that have prevented the student from attaining satisfactory academic progress and evidence that changes have occurred to allow the student to now meet standards of satisfactory academic progress. Only extraordinary circumstances will be considered, such as death or illness in the immediate family. Before an appeal may be granted, a written academic plan must be provided to the student which clearly identifies a viable plan for the student to complete the program within the maximum timeframe allowed.
The School Director will assess all appeals and determines whether the student may be permitted to continue in school or a probation status, despite not meeting the satisfactory progress requirements. The student will be sent the written decision within ten days of Fred’s Appliance Academy’s receipt of the appeal. The decision of the School Director is final.
Students reinstated upon appeal are on a probationary status for the next evaluation period, during which time they must meet the terms and conditions set out in the School Director’s letter granting the appeal. At the end of the evaluation period, and at every evaluation period thereafter, the student’s academic status will be reviewed. The student may continue on probation as long as he or she meets the terms of the academic plan approved at the time the student’s appeal was granted, until such time as satisfactory academic progress is regained. The student regains satisfactory progress status by meeting the minimum SAP standards.
TRANSFER AND READMITTED STUDENTS
Transfer students from outside Fred’s Appliance Academy will be evaluated qualitatively only on work completed while at the Academy. The maximum time frame is not reduced for transfer students. The maximum time frame may be reduced for readmitted students with Instructor and School Director approval.
MEDIA RELEASE FORM
I, hereby confirm the consent therefore given you with respect to your photographing/recording me in connection with websites, newspapers, magazines, DVD's, ETC, and I hereby grant to you, your successors, assigns and licensees the perpetual right to use, in any manner or in any media currently existing or which may be developed in the future, as they may desire, all video, still and motion pictures and soundtrack recordings and records which you may make of me or of my voice, and the right to use my name or likeness in or in connection with the exhibition, advertising, or any other use of such motion picture or recording. I understand that I may be provided with a copy of the websites, newspapers, magazines, DVD's, etc. for my personal use only. I will not seek compensation from the creators/producers for my involvement in this project. I will not sell
ELECTRONIC MEDIA
I, the undersigned, will not videotape or record the instructor, or the instructor's helpers, in full or in part, without explicit written permission from the instructor. If permission is granted, I agree not to share the recording with anyone or post the recording online, or in a public place for

FRED’S FLATS INCIDENTAL AGREEMENT
I understand that I'm responsible for leaving the flats in the condition in which it was issued to me. I agree to allow Fred's Appliance, LLC and/or Gene SR, LLC to charge my credit card on file in the event a loss is suffered or I do not leave the room/flat in proper condition. I also understand that there is no smoking allowed inside the flats. I agree to pay a $250.00 cleaning fee in the event I smoke in the flats. I understand that I am not permitted to tamper with the appliances. I agree to pay a $250.00 fee for doing so. I understand that I am responsible for damages that I have caused. I agree to pay the charges associated with those damages

In the event of an observed holiday, inclement weather, an instructors’ illness, or any other unforeseen circumstances that may cause interruption to normal class schedule a Saturday or Sunday may be used as a makeup day at the school’s discretion.
CANCELLATION POLICY
We reserve the right to cancel any session. Students will be notified of a canceled session on or before the 30th day prior to the session start date.

Registration Fees
All registrations incur a non-refundable $99.50 registration fee.
Processing Fees
Course fees are refundable, excluding $99.50 registration fee. We understand that the unexpected can happen, so if something comes up, just contact us; we will work with you.
Refund Policy
If the student is not accepted into the training program, all monies paid by the student shall be refunded. Refunds for books, supplies, and consumable fees shall be made in accordance with Ohio Administrative Code section 332-1-10.1. There is one (1) academic term for this program which is 120 clock hours in length. Refunds for tuition and refundable fees shall be made in accordance with the following provisions as established by Ohio Administrative Code 332-1-10.0:
(1) A student who withdraws before the first day of class and after the 5-day cancellation period shall be obligated for the registration fee.
(2) A student who starts class and withdraws before the academic term is 15% completed will be obligated for 25% of the tuition and refundable fees plus the registration fee.
(3) A student who starts class and withdraws before the academic term is 15% but before the academic term is 25% completed will be obligated for 50% of the tuition and refundable fees plus the registration fee.
(4) A student who starts class and withdraws after the academic term is 25% complete but before the academic term is 40% completed will be obligated for 75% of the tuition and refundable fees plus the registration fee.
(5) A student who starts class and withdraws after the academic term is 40% completed will not be entitled to a refund of the tuition and fees.
The school shall make the appropriate refund within thirty days of the date the student is able to determine that the student has withdrawn or has been terminated from a program. Refunds shall be based upon the last date of a student’s attendance or participation in an academic school activity.
Changing Your Course Dates
You may transfer to another class date instead of canceling. If you would like to transfer to another class, you must notify the Academy four or more days prior to the start of class. Failure to notify the Academy of your intention to transfer in four or more days prior to the start of class will result in a $99.50 processing fee. The first class transfer is free (provided that you give at least a 4-day notice), each subsequent transfer will incur a $25 processing fee.
